June 2026 President’s Letter

Dear Colleagues,

Welcome to my PES monthly President's Newsletters! I want to begin by expressing my heartfelt gratitude to Dr. Laurie Cohen for her outstanding leadership this past year. She is a role model for us all. I am deeply honored to step into this role and to work alongside our members in advancing PES's mission: "to advance and promote the endocrine health of children, adolescents and young adults and support the professionals that work on their behalf."

June always carries a special energy with warmer days, a sense of culmination, and the bittersweet joy of watching our trainees move forward. To our graduating fellows: congratulations, you did it! After four years of college, four years of medical school, three years of residency, and three years of fellowship, you have earned every bit of this moment. Please take time to reflect on how far you've come, not just in clinical knowledge and professional skills, but as people. You and your families deserve to celebrate wholeheartedly.

I still vividly remember my first day as an attending. After the intern presented the patient on rounds, the case manager looked directly at me and asked, "So, the patient is ready for discharge?" There was no one behind me, no one to call. The decision was mine. That was the moment I understood that the responsibility to advocate for the best care now rested with me. What makes this decision less daunting is that in pediatric endocrinology we never make these decisions alone.  We lean on each other: our colleagues, trainees, nurses, CDEs, and staff, working together in advocating for our patients.

And this month, that collective voice matters more than ever. As a professional society of physicians, advanced practice providers, educators, and researchers, we must advocate not only for excellent clinical care, but for the preservation of pediatric research and the therapies our patients depend on. Our years of training give us the expertise to know what is best for our patients and their families and our shared commitment to improve that care. We cannot allow those without medical training to defund our studies, obstruct our innovation, or restrict lifesaving therapies. I encourage every member to submit comments on the proposed Regulations for Federal Financial Assistance by July 13, 2026. All children, adolescents, and young adults deserve quality healthcare, and we must be their voice.

Until next month, I hope you find time to enjoy the warm days ahead; you've earned them too.

Warmly,

Tandy Aye, MD
President, PES Board of Directors

Industry Sponsored Symposium: Shifting The T1D Paradigm: Early Detection, Disease Modification, and Beta-Cell Preservation

With the advent of disease-modifying therapies to delay progression of type 1 diabetes (T1D), as well as their potential application of these therapies in newly diagnosed T1D, there is an increased emphasis on presymptomatic screening, staging, and monitoring—representing a true paradigm shift in how we address the disease. This proactive approach is paramount in pediatric populations, who are at higher risk for progression after testing positive for islet autoantibodies. This symposium will provide an overview of the practical considerations for early detection of T1D and the application of disease-modifying therapy for delaying disease progression in pediatric patients, while also exploring emerging treatment approaches aimed at preserving beta cell function in newly diagnosed T1D.

Speakers:

Daniel DeSalvo, MD, Associate Professor, Pediatric Diabetes & Endocrinology, Baylor College of Medicine, Texas Children's Hospital, Director of Strategic Collaboration, Texas Children's Diabetes Care Center, Houston, Texas

Roshanak Monzavi, MD, Clinical Professor of Pediatrics, Director of Fellowship Program, Center for Endocrinology, Diabetes, and Metabolism, Children’s Hospital Los Angeles, Los Angeles, California

Learning objectives:

  • Implement risk-based screening approaches to facilitate timely identification of presymptomatic T1D
  • Analyze T1D screening results to determine disease stage and inform appropriate monitoring and referral decisions
  • Develop individualized treatment strategies that incorporate disease-modifying therapies to delay T1D progression in eligible patients
  • Recognize the importance of beta-cell preservation on T1D prognosis and outcomes
  • Review emerging approaches aimed at beta cell preservation & their potential future application in new-onset T1D

Advocacy Spotlight: Call to Action - Regulation for Federal Financial Assistance

The Office of Management and Budget (OMB) has released a proposed rule, Federal Register: Regulation for Federal Financial Assistance, that would make broad changes to the regulations governing federal grants and other forms of federal funding. The proposal includes provisions that may affect pediatric endocrine research, training programs, and academic medical centers, including policies related to award selection, suspension and termination of awards, nondiscrimination requirements, and other administrative requirements for federal funding recipients.

The Research Affairs Committee has reviewed the proposal and submitted comments on behalf of PES. We encourage members to review the proposed rule Federal Register: Regulation for Federal Financial Assistance, and, if interested, submit comments based on their own expertise and experience.  Comments are due July 13, 2026.
